A TELEPHONE service where nurses offer help, advice and support is making a big improvement to patients’ lives.
Telephonic health services in Barnsley are run by the South West Yorkshire Partnership NHS Foundation Trust (SWYPFT) which has a team of team of nurses to help people who have long term illnesses.
There are three elements to the service; care navigation, which offers advice and directs people to the best service for them; health coaching, where people set a goal such as losing weight or regular exercise and have frequent contact with a member of staff to help them achieve that goal; and vital sign monitoring, where people with conditions such as emphysema and heart disease are monitored.
